Steps to Become an Unfading Beauty Premiere Clinic/Spa/Salon

1. Complete our easy online form. It only takes a couple minutes.

2. An Unfading Beauty Clinical Skin Care representative will call you to process your request for registration.

3. Submit the following documentation:
- A photo copy of the license of each practitioner
- A photo copy of the facility license
- A photo copy of current liability insurance, this must be resubmitted annually
- Your retail sales tax identification number

4. Agree to and sign the Unfading Beauty Clinical Skin Care Confidentiality Agreement. A copy must be submitted for our records.

5. Complete training and certification on all products with a pH of under 3 through UB, Inc. only.
